In an effort to cut costs we have included the Athletic Paperwork Packet as a download for your convenience; this must be completed and turned in for your child to participate in our sports program.

Please Note: You only need to print pages 1 to 16 - (there are 28) if you do it in the library you must pay $.05 for each page printed. 16 pages costs $.80 - 28 pages costs $1.40

You must also print out the Concussion Information Sheet and have it signed, and turned in too. Print both pages. This is new for 2012.
