Please Note: You only need to print pages 1-16 - (of 28). If you print in the library, it costs $.05 for each page printed, so 16 pages costs $.80 and 28 pages costs $1.40.

You must also print out the Concussion Information Sheet and have it signed, and turned in too. Print both pages. This is new for 2012.
