Please Note: You only need to print pages 1-16 (out of 28). If you use the library printer, you must pay $.05 for each page printed; 16 pages is $.80, while all 28 pages costs $1.40.

You must also print out the Concussion Information Sheet and have it signed, and turned in too. Print both pages. This is new for 2012.
